You are eligible for a HSA if the following conditions are met:

• You cannot be claimed as dependent on someone else's tax return.
• You may not be covered by any other health insurance.
• You must not be enrolled in Medicare.
• You must be covered by a qualifying High Deductible Health Plan.
• Annual Contribution Limits apply.

You will need the information below to get started. Once you have submitted your information, a banker will contact you within 1 business day to complete opening your HSA account. 

• Type of plan (Individual or Family) 
• Contribution Type (Transfer, Rollover, etc.) 
• Contribution Year (Current or Prior Year)
• List of Beneficiaries (if applicable)
